Monitoramento da covid-19 a partir da análise do esgoto é destaque em Sessão Especial da Brazil Water Week

Especialistas dos Estados Unidos, Reino Unido e Brasil apresentaram as metodologias utilizadas para a vigilância epidemiológica da Covid-19 por meio do monitoramento das águas residuais.

Nesta terça-feira, 17 de maio, a terceira Sessão Especial rumo à da Brazil Water Week – Semana da Água do Brasil, que começa nesta segunda (23), trouxe para o debate como o monitoramento do esgoto foi utilizado como uma ferramenta de vigilância epidemiológica da Covid-19 no Brasil e no mundo. Realizado pela Associação Brasileira de Engenharia Sanitária e Ambiental – ABES e transmitido gratuitamente online, o webinar contou com a presença de especialistas dos Estados Unidos, Reino Unido e Brasil. 

O gerente de pesquisa e inovação da Sanepar, coordenador do Câmara Temática de Tratamento de Esgoto da ABES e diretor da ABES-PR, Gustavo Rafael Collere Possetti, coordenou e apresentou a sessão “Monitoramento do esgoto como ferramenta de vigilância epidemiológica da Covid-19: experiências mundiais e conhecimentos à serviços da população”. O debate teve como convidados Angela Harris, professora da Universidade Estadual da Carolina do Norte, nos Estados Unidos; Hubert Denise, chefe de genômica no setor de monitoramento ambiental para proteção da saúde da Agência de Segurança de Saúde do Reino Unido; e César Mota, professor da UFMG, coordenador do INCT ETEs Sustentáveis e da Rede Monitoramento Covid Esgotos. A sessão contou com a moderação de Thais Araújo Cavendish, coordenadora geral de vigilância em saúde ambiental do Ministério da Saúde.

A doutora Angela Harris é professora assistente no Departamento de Engenharia Civil, Construção e Meio Ambiente e membro do Global WaSH Research Cluster da Universidade Estadual da Carolina do Norte. A sua pesquisa se concentra amplamente na transmissão ambiental de doenças infecciosas e no desenvolvimento e avaliação de estratégias relacionadas à água, saneamento e higiene (WASH) para proteger a saúde humana. Durante a sessão, Harris apresentou o método utilizado para medir a propagação do vírus covid-19 utilizando as águas residuais do estado, dados que ajudaram o departamento de saúde da Carolina do Norte a prever onde os surtos poderiam ocorrer e a instalar políticas públicas de combate ao vírus.

“A vigilância de doenças é muito importante para determinarmos como alocar os recursos e para entendermos até que ponto as doenças estão presentes em uma população. E é muito importante para a tomada de decisões, como por exemplo, a criação de ordens para ficar em casa, ou a localização de acesso a testes e também para identificar fontes em potencial ou situações em que as doenças estão se disseminando”, afirmou a pesquisadora, ressaltando a importância do monitoramento de doenças para a saúde pública. Ela apresentou a forma como as pesquisas anteriores de monitoramento de doenças foram adaptadas desde o início da pandemia para a eficaz detecção do SARS-CoV-2.

De acordo com a professora, o monitoramento de esgoto teve uma contribuição importante, na Carolina do Norte, como um dos muitos dados utilizados para fundamentar decisões complexas quanto ao combate do novo coronavírus, como o lockdown e o uso de máscaras, “e essas são decisões de alto cunho político também, então é útil que haja mais dados para que se tomem decisões melhores”.

O segundo convidado, Hubert Denise, é líder operacional de genómica no setor de monitoramento ambiental para proteção da saúde na Agência de Segurança da Saúde do Reino Unido. Ele começou em Bioinformática enquanto trabalhava para a Pfizer no desenvolvimento terapêutico de RNAi contra o vírus da hepatite C.

O especialista falou sobre como a genômica foi utilizada para o monitoramento da covid-19, cobrindo a maior parte da população inglesa pelo estudo de água residual. No Reino Unido, amostras de esgoto foram coletadas em todas as regiões várias vezes por semana e foram utilizadas no estudo e através do PCR, puderam detectar e sequenciar o vírus em cada região durante esses dois anos de pandemia.  Através da bioinformática, as variantes do vírus também puderam ser detectadas.  Em comparação com os testes clínicos, as detecções pelo esgoto estavam sincronizadas e até mesmo, antecipavam levemente as detecções clínicas. 

O terceiro convidado apresentou o Projeto-piloto Covid Esgotos, uma iniciativa conjunta da Agência Nacional de Águas e Saneamento Básico (ANA) e do Instituto Nacional de Ciência e Tecnologia em Estações Sustentáveis de Tratamento de Esgoto (INCT ETEs Sustentáveis – UFMG), em parceria com a Companhia de Saneamento de Minas Gerais (COPASA), o Instituto Mineiro de Gestão das Águas (IGAM) e a Secretaria de Estado de Saúde de Minas Gerais (SES), para o monitoramento da presença do vírus SARS-CoV-2 no esgoto sanitário nas cidades de Belo Horizonte e Contagem, Minas Gerais. O Dr. Cesar Mota é PhD em Engenharia Civil e Ambiental pela Universidade Estadual da Carolina do Norte, nos Estados Unidos, e hoje é professor associado do Departamento de Engenharia Sanitária e Ambiental da Universidade Federal de Minas Gerais (UFMG), no Brasil.

“O esgoto pode ser uma fonte de doenças, mas também pode ser uma fonte de recursos, pode ser uma ferramenta epidemiológica muito valiosa que ainda temos muito a aprender”, afirmou o professor.

César Mota salientou que, durante a pandemia, em Belo Horizonte, os dados coletados sobre a covid-19 são semanalmente discutidos nas reuniões do comitê de enfrentamento municipal e estadual, além de serem discutidos também pelas autoridades que gestam o Aeroporto Internacional de Confins.  “E também, sempre que detectamos alguma amostra positiva no lar de idosos monitorado por nós, imediatamente notificamos por e-mail a prefeitura responsável por esse lar e eles usam esses dados para realizarem a testagem desses idosos”.

Ao mediar o debate, Thais Araújo Cavendish, analista técnica de Políticas Sociais no Ministério da Saúde desde 2013, com atuação em Vigilância em Saúde Ambiental, comentou sobre as falas dos três participantes. “A gente viu, na fala dos três palestrantes, a relevância e a praticidade que se tem ao usar a metodologia baseada no esgoto quando a população tem acesso, ou seja, é conectada à rede de esgotos do município ou região”, disse, ressaltando que o acesso ao saneamento facilita o trabalho de pesquisa de monitoramento de vírus. “Portanto, o abastecimento de esgoto para a população retorna para a sociedade como uma ferramenta e fonte de informação riquíssima sobre a saúde”.

“É importante perceber o quanto estas detecções estavam em sintonia com os indicadores clínicos de testagem, mas também o quanto estavam até antecipados em relação aos indicadores clínicos observados em todos os cenários epidemiológicos dos países apresentados aqui”, enfatizou a representante do Ministério da Saúde brasileiro.
